In order to be considered a registered parishioner, an adult (persons 21 years of age or older) must submit a completed registration form. These forms can be printed or picked up at the parish office. Completed forms must be brought to the parish in person. Please remind young adults (21 and older) that they must be separately registered with the parish.
Ordinarily, for Baptism as an infant, for First Holy Communion and for Confirmation, the family must be registered as parishioners of the parish.
For Matrimony, usually at least the bride or the groom must be a registered for at least six months.
To receive a certificate of eligibility to serve as a godparent or a sponsor for Confirmation, an individual must be a registered parishioner for at least six months and regularly attend Sunday Mass here at OLMV. The individual seeking to be a godparent or sponsor must complete a request form and personally submit it to the parish office. There are other requirements as well. Please consult the Bulletin or the parish office for further information.
